This template is a simple and direct way to provide your customer with a description and photographic evidence of work completed. 
 
The form will show the description on the first page and the photos starting from the second page, but the page break can be easily removed if you prefer.
 
Job attachments must be marked as Public to appear on the form. 
 
This template is a neutral theme, in black and white and Arial font, ready to have your own branding, theme and colours applied.
